Membership Tier PricingPractice Model
To maintain our highly personalized, limited-panel model, Dr. Long requires a comprehensive 1-on-1 virtual clinical evaluation prior to establishing formal membership. A one-time $250 consultation fee is processed securely via email link to officially confirm your scheduled day and time slot. If you are accepted into the practice and enroll in an annual membership during your consult, 100% of this fee ($250) will be credited directly toward your first month's invoice.
Payment Structure
While we do not accept insurance for membership fees or compounded medications, you can use insurance for retail medications and labs (through Quest). In addition, HSA/FSA may cover up to $150/month with a medical necessity letter.

How do I book an appointment?
Go to "New Patient Consultation" to set up your first consult. If you just have a question, go to the "Contact" section to send an email.
What should I know before my first visit?
What are your health goals? Be prepared to discuss your medical history, what your current health goals are, and if this will be a good fit for your lifestyle.
How long do video visits take?
Typically, 30 to 60 minutes
Do you offer monthly memberships?
Annual memberships are preferred. However, monthly fee memberships are offered with a 1-year contract.
Do you take insurance?
While insurance cannot be used to pay for your membership, it can be used for retail prescriptions and lab services. Any compound medications will not be covered. Your HSA may cover some of your costs with a medical necessity letter.
What is your cancellation policy?
If you cancel within 24 hours, 50% of the fee will be held. If you cancel more than 24 hours in advance, you can request a refund.
